Wayne Center

Description:

Wayne Center is a non-profit human services agency with a 46 year history of providing supports coordination and clinical support to those persons with intellectual and developmental disabilities. Wayne Center believes that the individuals we serve will attain their full self- determination, rights of citizenship and economic and social benefits. We assist them as they advocate on behalf of themselves; and evaluate and continually improve services which support their empowerment and self-advocacy.

OUR SERVICES
Our services are person centered and designed with maximum control by people with disabling conditions and other behavioral care needs. They are individualized and relevant to individual needs and desires. We strive to make our services culturally diverse and change based on the need of an individual. Our goal is to help individuals achieve maximum independence. We measure our success in the quality of the satisfaction of the person served.
  • Advocacy:
  • Wayne Center’s Board of Directors and staff act on behalf of our consumers to ensure individual development and fulfillment of life.
  • Behavioral Training
  • Providing the training for all direct care staff, including group and nursing home employees to deal with challenges and behaviors of our consumers.
  • Supports Coordination:
  • Working with the consumer utilizing person centered processes to identify and implement support strategies to assure ongoing satisfaction with supports, services and available resources.
  • Group Home Staff Training:
  • Provide training to direct care staff on consumer support plans, recipient rights, and working with individuals with disabilities.
  • Diagnosis and Evaluation:
  • Provide comprehensive needs assessment for each consumer to evaluate placement needs and recommendations for services.
  • Individual Therapy:
  • Provide individual therapy to meet the needs of the consumer.
  • Home Development:
  • Identify potential home providers, assist with licensure issues and place consumers in appropriate settings.
  • Individual Health Care Planning:
  • Provide a comprehensive health assessment to formulate a health care plan to meet health needs.
  • Permanency Planning:
  • Provide for stable and consistent living arrangements that will enhance growth and development.
  • Placement:
  • Appropriate placement to address the needs and desires of the consumer in various residential settings.
  • Psychiatric Consultation:
  • Provide for psychiatric evaluation and periodic review of each consumer.
  • Medication Review:
  • Monthly review of consumer medication to check for effectiveness and side effects.
  • Person Centered Planning:
  • A planned approach of service and supports determined by the consumer and facilitated by a network of service providers, family and friends.
  • Psychological Supports:
  • Provide psychological assessments and psychological consultative and in-service functions.
  • Referrals:
  • Manage referrals with other agencies and service providers to ensure that every possible advantage is secured for consumers.
  • Transitional Training:
  • Individualized support and guidance to facilitate movement to a least restrictive residential setting designed to create and maintain independence
