Community Connections for Independent Living

Description:

Our staff looks at our residents as a whole person, not just their injuries.
The CCFIL staff is committed to finding activities that work with their rehabilitation plan instead of making them participate in pre-set activities just to keep them busy.
Our program is anything, but rigid. If something works for one of our residents, we will often try it with the next.  However, if it doesn’t work for them, we come up with a new game plan. Together, our staff is always looking for new things for our residents to be involved in.
We believe in letting residents figure out for themselves what they need and will continually reevaluate to make sure they always stay on the path of progress. No matter what, we always approach their deficits with compassion and work as an entire team to help them come to terms with where they are at on their journey to recovery.
At the end of the day, we will do what it takes to help our residents build the confidence they need to continue to move forward.
An Individualized rehabilitation program (IRP) addressing 5 life domains, cognition, and prosocial skills.
A professional, experienced staff that includes a program director, medical director, registered nurses, occupational therapist, and social worker
At a minimum 8-10 hours of professional staff contact per month to provide ongoing support and development of IRP goals and to evaluate the residence progress.
At a minimum, 6-8 hours of contact with community connections rehabilitation coaching staff per week to monitor and support IRP. These sessions are 1 on 1 between the resident and staff member.
At a minimum, 2-3 hours per week of structured functional training sessions under the direction of the program director or occupational therapist to address identified functional deficits, in the areas of:
  • Information Processing
  • Visual Processing
  • Memory, Attention and Executive Functioning
  • 2 to 4 recreational and social opportunities per month at request of the residents.
  • Job Placement Support and Job Coaching, at the request of the resident.
  • Scheduled Team Meetings
  • Bi-Monthly Progress Reports
  • A furnished apartment
  • 24-hour gated security
  • All utilities
  • Land line phone
  • Cell phone for off-site communication
*Additional professional therapies, if needed, will be provided by Community
