Community Living Supports
The Community Living Program focuses on supporting individuals with a developmental disability, traumatic head injury, mental illness, mobility issues or any of the problems associated with aging including Alzheimer’s disease. We support these individuals’ choice to live independently in their own homes or apartments. We are committed to providing the support and/or training necessary to make that independence possible and maximize their life experiences. CHS believes that all people achieve their full potential through self esteem, community inclusion and independent living. Our services include but are not limited to:

Skill Building/Day Program
The purpose of our Skill Building Day Program is to provide purposeful and meaningful activities to support and promote each person served with skills to lead them to greater independence in the future.
Skill building at the CHS gives individuals the opportunity to work a small groups with an instructor in the community to develop and enhance skills related to:
- Volunteerism
- Employment Skills
- Independent Living
- Health and Wellness
Supported Employment
People with intellectual and physical challenges have many talents and skills that are overlooked. For over a decade, our clients have been showing employers just how much they have to offer! CHS promotes career exploration and job shadowing so that individuals with limited experiences are able to gain an understanding of what is available in the job market. In addition, our customers are carefully assessed so that they can identify positions that will best match their skills and interests.
Our services support the individual through the entire employment process. We assist with job seeking skills, employment placement and on the job training. In some instances, a job coach may be assigned to help ensure that successful work skills are developed and reinforced.
